CDM Boys Summer Camp 8/4-8/8 2-4 PM

In return for being permitted to participate in CDM Boys Volleyball Summer Camp at Corona del Mar High School, the UNDERSIGNED hereby RELEASES, WAIVES, AND FOREVER DISCHARGES , the CDM Boys Volleyball Foundation (AKA: BOOSTERS/FOUNDATION), its members, coaches, staff, volunteers, or and other program participants and their families from liability from any and all claims, including the negligence of the CDM Boys Volleyball Summer Camp, resulting in personal injury, death, accident or illness, and property loss, in connection with participation in the CDM Boys Volleyball Summer Camp at and any use of the premises and facilities. INDEMNITY AND HOLD HARMLESS: I hereby agrees to indemnify and hold the CDM Boys Volleyball Foundation harmless from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of the undersigned’s involvement in the CDM Boys Volleyball Summer Camp, and to reimburse the FOUNDATION for any such expenses incurred.
